FSC Rental Order Process

1.�� Determine which phone kit/accessories you need
For assistance please view the Rental Flyer (pdf) or contact the rental department directly at rentals@fsclp.com or 801-754-1621 Ext 6

2.�� Download, print, and complete the Rental Agreement (pdf)

3.�� Fax in Rental Agreement to 801-754-1721
Make sure to include a valid email address on rental agreement

THEN

4.�� Order confirmation will follow via email from the rental department
If you do not receive a confirmation email please contact the rental department at 801-754-1621 Ext 6 to ensure order receipt and fulfillment

5.�� Rental equipment will arrive via FedEx or UPS, unless otherwise arranged, the business day before the rental start date

6.�� Upon receipt, customers must make a test call to FSC's toll-free service line, 6111, to ensure functionality of the unit

7.�� Rental Equipment must return to FSC within one business day of the rental end date
Equipment received after this grace period will be subject to additional daily rental fees. Customers may choose the carrier for return shipment of equipment

Rental Billing Process

1.�� Online Charges for rental equipment, shipping/handling, and loss/damage coverage protection (optional) will bill to the credit card immediately. Faxed orders will bill up to 5 days prior to order shipment/pick up

2.�� Charges for rental airtime will bill to the account provided the first month after use
